To Get Started with Our New Client Form:

Please call us at (760) 466-5216 to set up your appointment. To expedite your check-in, once the appointment is created, we will send you our New Client form for you to complete at home. If you are a new client or coming in for an annual exam, we kindly ask that you arrive 20 minutes early. Your pet(s) will need an examination by one of our doctors to be considered established clients. Thank you for your understanding!

Adding a Pet?

Please call us at (760) 466-5216 so that we can add your additional pet to our system. Once added, we will send you the Additional Pet Form to complete at home.
