New Patient Center

** For all potential new clientele at this time we are not accepting new clients and please ask for all future new clientele to check back with us in the beginning of new year.

Please be aware that we require our future new clientele to complete our "New Client Registration Form" a minimum of 24 hours prior to your pets appointment. This allows our staff to input your information into our system before the scheduled appointment.
 
If the paperwork is not completed
24 hours before your appointment you will be asked to complete it at the clinic. If you are unable to complete the forms online, please stop by our clinic to receive a hard copy of the forms listed below.

We are prioritizing appointments to be escorted into the Clinic and will schedule non-urgent visits (wellness checks, routine senior appointments, and elective procedures when our schedule permits. Our focus and priority will be current clienteles' pets with urgent health issues.

To reduce the spread of the coronavirus, you will see safety protocols in place at Ridgeline Veterinary Clinic. Face Masks will continue to be worn by the staff members and clientele at Ridgeline Veterinary Clinic even if you are fully vaccinated until further notice.
Please remain in your vehicle upon arriving at the clinic. A staff member we'll meet you curbside at your vehicle to assist you. See our "COVID-19 Policies" for more information.

To register with the clinic for future appointments Ridgeline Veterinary Clinic offers our "New Client/Patient Form" online so you can complete it in the convenience of your own home or office. This will establish your file into our management software ready for future appointments so that we can effectively and efficiently provide the service you deserve!

  • The clinic's online forms through pdfFiller below are simple, step-by-step process, and pdfFiller will walk you through it! 

  • Click the pdfFiller icon below for the desired form. Once completed and signed, the form will be directly emailed to us when you click the “DONE” tab in the upper right corner of your screen. After completion you will be prompted to an "All Done, Thank You Notification" that the documentation has been emailed to us. If you have more than one pet please fill out the separate form "Additional Pet Form".

  • PATIENT RECORDSPlease fax or email your pets medical records to our clinic.

  • PRIOR VETERINARIAN PATIENT RECORDS - Please contact your pets prior veterinarian to give verbal consent to have your pets' medical records faxed or emailed to our clinic.

  • Please call the clinic after completing the form(s) to schedule an appointment. Please leave us a message if you are unable to reach us and one of our receptionists will return your call as soon as possible. We currently do not schedule/reschedule appointments via email.
